The hospital was a multistoried building. It offered a variety of treatment methods and departments, including an oncology ward. The patient rooms were located on the clinic's upper levels. Additionally, there was a cafeteria frequented by the patients, staff members, and visitors. (Saw: Rebirth)

Throughout its history, countless patients have been treated at the hospital. One of them was John Kramer, a toymaker who worked as a designer in the toy division of Standard Engineering Ltd. When he went to the hospital, the oncologist Dr. Lawrence Gordon diagnosed him with a malignant tumor. Although it was not far advanced, John suffered from a particularly virulent strain of cancer. When telling him about his diagnosis, Gordon clarified that he was not expecting his patient to survive. After John received his terminal diagnosis, Zep Hindle, an orderly, talked to him and openly showed his disdain for the doctors' indifferent attitude toward their patients. He claimed they were all cheating on their wives and that he would not be like them once he became a doctor.

During his hospital stay, John began focusing on other people’s lives. While wandering around, he encountered a scam artist named Mark, who talked to the other patients, possibly attempting to defraud them. Another patient he met was Amanda Young, a heroin addict who almost died of an overdose. However, none of these events shocked him as much as seeing Paul, one of his colleagues from the toy factory, who had unsuccessfully tried to kill himself by cutting his wrists. Although the doctors urgently appealed to him to stay at the hospital for an undetermined time, John interrupted his treatment and returned home. (Saw: Rebirth)
