The Brain Surgery Trap is a trap in Saw X.
Design and Function[]
This trap takes place in an abandoned chemical factory, once used as a base by Cecilia Pederson to conduct her fake surgical operations on terminally ill cancer patients. Mateo is the second con-doctor to be tested in this game.
The design has the victim strapped to a defunct wheelchair being held down by a collar around their neck. A key to remove himself from the trap is in a locked container on the table in front of them. A pneumatic trough is also on the table, and when brain matter is placed in the glass, it will dissolve to release the key and allow the victim to free themself from the trap. The victim also has an electrical collar and band around their head, to administer shocks when required by Jigsaw. An open mask is positioned around the subject's face, which is wrapped in heat coils, which will slam shut and kill the victim if the task is not completed in time. A camera is also placed above the victim to allow them to see their head through a monitor during the procedure.
The victim is given 3 minutes to obtain cerebral tissue from their brain and put it in the trough placed in front of them, which completes the electrical circuit and releases the key that allows the victim to escape. The victim is given tools such as a bone cutter, forceps, and more medical supplies to complete the task.

After witnessing the death of his coworker Valentina, Mateo is tazed unconscious by Amanda. Mateo then wakes up inside the Brain Surgery Trap with a round hole at the top of his head, partially revealing his skull. He reluctantly plays the tape received by Billy on his tricycle after being shocked by the electrical collar around his neck for initially refusing to play the game. He's told that he has three minutes to place a portion of his cerebral tissue into the trough to complete a circuit that releases the key to the device strapping him to the chair. The device also shocks Mateo when he isn't completing the trap.
The timer begins, and though hesitant at first, Mateo plugs in the craniotome and begins cutting away at his skull. He successfully cuts off the exposed skull area and reveals his brain matter. He uses a pair of forceps to rip the brain matter from his skull with less than 20 seconds left. Unfortunately, he doesn't obtain the key in time, as it takes too long for the brain matter to dissolve. With his three minutes up, the heat coils in the mask activate, glowing bright red at full heat, much to Mateo's horror. As he screams, the mask closes and burns Mateo's head, killing him.
Trivia[]
- This trap is influenced by an unused trap from Saw II, where Addison was supposedly meant to have a trap that would result in her face being melted off, made from a waffle maker. Its closing mechanism also very much resembles the Death Mask, also from Saw II.
- An image of the mask when closed was used as a profile picture by Lionsgate for a brief period on social media and on the Snapchat filter of the trap.
- The mask is based on Tlāloc, the Aztec God of rain, hail, thunder, and lightning.
- Originally, Mateo's face after being killed from his trap from failing was suppose to be shown.
